Online Help – Verify Credentials

Introduction

Midas+ Seeker Refer & Verify is a browser-based application used for medical or professional staff member referral or verification. The appearance of this system depends how it was tailored to your organization’s needs. For example, your organization chooses what options, fields, field labels, and button labels are displayed. This general Help describes what might be displayed in your unique version of Seeker Refer & Verify.

Verify Credentials

Note:  * = mandatory field entry

To verify the clinical privileges or medical staff memberships of a medical or professional staff member and compose a verification letter, follow these steps:

1.       Click the Verify Credentials link. The Verifying a Medical/Professional Staff Member page is displayed.

2.       Enter search criteria into any of the following fields that might be displayed:

Last Name Type the last name or the first few letters of the last name of the medical or professional staff member.

First Name Type the first name or the first few letters of the first name of the medical or professional staff member.

Organization From the drop-down list, select the organization where the medical or professional staff member has privileges. You can also type the first few letters of the organization to display and select it.

Specialty From the drop-down list, retain the Any Specialty default selection or select the medical or professional staff member’s specialty. You can also type the first few letters of the specialty to display and select it.

Birthdate mm/dd/yy Type the medical or professional staff member’s birthdate in mm/dd/yyyy format.

SSN Type the last four digits of the medical or professional staff member’s Social Security Number.

3.       Click the Submit Query button. The Search Results for Privileges page is displayed. Results for the provider or professional staff who met the criteria you entered are displayed in the Name, Specialties, Appt Date, and End Date fields.

4.       Click a medical or professional staff member’s name. The Verification Requestor Information form is displayed.

5.       Enter the following information into any of the following fields that might be displayed:

Requestor Title  Select the verification letter requestor’s title from the drop-down list.

Requestor Name Type the verification letter requestor’s name in <First Name Last Name> format. If the requestor’s name has been entered before, you can type the first few letters of the name to display and select it.

Job Title Type the verification letter requestor’s job title. If the requestor’s job title has been entered before, you can type the first few letters of the title to display and select it.

Company Name Type the name of the company represented by the verification letter requestor. If the company name has been entered before, you can type the first few letters of the name to display and select it.

Address (line 1) Type the street number and name in the company’s address. If the address has been entered before, you can type the first few numbers or letters of the address to display and select it.

Address (line 2) If a second address field is displayed, if needed, type additional address information (for example, suite number) in the company’s address. If the address information has been entered before, you can type the first few numbers or letters of the information to display and select it.

City Type the name of the city in the company’s address. If the city name has been entered before, you can type the first few letters of the name to display and select it.

State From the drop-down list, select the name of the state in the company’s address. If the state name has been entered before, you can type the first few letters of the name to display and select it.

Zip Code Type the zip code in the company’s address. If the zip code has been entered before, you can type the first few number of the zip code to display and select it.

6.       Do one of the following:

·         Read any liability release statement that might be displayed. Click the Submit button to acknowledge you agree with the statement and to submit the verification request. The verification letter for the selected medical or professional staff member is displayed for viewing and printing. Click the browser Print button to print the letter.

·         Click the Back to Results button if you don’t want to submit the verification request. The Search Results for Privileges page is displayed. You can either click another medical or professional staff member’s name and submit another verification request, or click the Back to Search button to return to the Verifying a Medical/Professional Staff Member page.


7.       Click the Submit Query button. The Medical/Professional Staff Search Results page is displayed. Results for the provider or professional staff who met the criteria you entered are displayed in the Name, Specialty, Organization, and Office Location fields.

8.       Click the provider or staff member’s name to display details such as office locations, office phone numbers, hospital location, specialties, languages, affiliations, education and training, board certifications and statuses, and professional memberships.

9.       You can click the Map it button to display a map of each office location.

10.   Click the Back to Results button to return to the Medical/Professional Staff Search Results page.

11.   Click the Back to Search button to return to the Locating a Medical/Professional Staff Member page.
